CLIENT ACCOUNT UPDATE


Receive important notifications, advisories and the latest updates from Philequity Management, Inc. 

Follow the steps below to keep your information and contact details up-to-date:

  1. Accomplish the forms
  2. For Individual or Joint Accounts:

    1. Fill out an Account Update Form
    2. Make a copy of a valid government issued ID
    3. Proof of latest billing statement that matches your present/permanent address on the Account Update Form
    4. Proof of Bank Account with complete account name and number that matches your settlement bank details on the Account Update Form.

    Additional Requirements for In-Trust-For (ITF) Accounts

    1. In Trust For Account Form
    2. PSA Birth Certificate of beneficiary

    For Corporate Accounts:

    1. Fill out an Account Update Form
    2. Make a copy of a valid government issued ID of each signatory
    3. Make a copy of latest General Information Sheet (GIS)
  3. Send the forms and supporting documents via email to ask@philequity.net
